Our Nursing Home Transitions
If you or a family member want to leave an institution, such as a nursing home, to pursue a more independent life, this program is for you. Nursing Home Transitions is a program that assists individuals with disabilities in transitioning from nursing facilities into the community, as well as providing services to assist in avoiding a nursing home readmission. Face it, living in a nursing home is not ideal.
Personal choice is limited, freedoms are limited, life is limited. Let CORD help you realize what you can obtain and enjoy the freedoms and life that you had before you moved into the nursing home. CORD’s Transition Coordinator assists in accessing only those services that you want and will help you move to the setting of your choice. It’s your life. YOU should decide what you want.
